Official Rules – Nationwide $10 Quote Reward Offer
These are the Official Rules for the Nationwide Quote Reward Offer ("Promotion"). The Promotion is sponsored by Nationwide Mutual Insurance Company ("Nationwide"), One Nationwide Plaza, Columbus, Ohio 43215.
No purchase is necessary to participate. Making a purchase will not increase the odds of receiving a reward.
1. Eligibility.
1.1 Invitation Required. The Promotion is by email invitation only and applies only to the owner of the email address that receives the quote reward offer email.
1.2. Open to Legal Residents. The Promotion is open to legal residents of the fifty United States and the District of Columbia, except where excluded and prohibited by law, who are 18 years of age or have reached the age of majority in their jurisdiction of residence at the time of promotion invitation, whichever is greater. Excluding the states of Alaska, Colorado, Connecticut, Delaware, Hawaii, Kansas, Massachusetts, Maine, Vermont and Washington. No purchase is necessary.
1.3. Nationwide Excluded. Employees and agents of Nationwide, its affiliates, subsidiaries, advertising and promotion agencies, and any entity involved in the development, production, implementation, administration, or fulfillment of the Promotion (collectively referred to as "Promotion Entities") and their immediate family members and/or those living in the same household of such persons, whether related or not, are not eligible to enter the Promotion.
1.4 No Other Sponsorship. The Promotion is not sponsored, endorsed, administered by, or associated with Tango Card, Inc.
2. How to Participate.
2.1. Invitation Process. Offer recipients will be selected by Nationwide. Nationwide will send invitations by email with instructions for redeeming their quote reward offer.
2.2 Criteria for Reward Offer. Whether a person receives a reward is contingent upon fulfilling all requirements set herein. Offer recipients must use the single-use link provided in the quote offer reward email to access the promotion landing page and start a quote for a canine or feline Nationwide pet insurance plan. Quote must be completed, as determined by Nationwide in its sole discretion, to receive any reward. A quote is considered complete after all required information is received including: zip code, pet information, email address and pet insurance plan selection. Email address used in quoting process must match the email address that received the quote reward offer to qualify for a reward.
2.3. Restrictions. Offer can only be redeemed online using the quote offer reward email link. Limit one (1) reward per person, household, email address and/or username will be allowed in any twelve-month period. Rewards limited to the first 1,000 eligible offer redemptions.
2.4. Conditions. Nationwide reserves the right to disqualify participants, at their discretion, for reasons, including, but not limited to:
a. Any attempt to obtain more than the stated number of rewards by using multiple/different email addresses, identities, registrations, logins, or any other methods;
b. uses AI or automated systems to participate;
c. infringes any third party's Intellectual Property Rights, privacy rights, publicity rights, or other personal or proprietary rights;
d. contains any information or content you do not have a right to make available under any law or contractual or fiduciary relations; or
e. is fraudulent, false, misleading, or deceptive.
2.5. No Withdrawal. Once submitted, participants cannot be changed or withdrawn from the Promotion.
2.6. Notice of Termination. Should the Promotion be terminated before the stated expiration date, Nationwide will notify eligible offer recipients by email.
3. Reward Recipients.
The first 1,000 eligible offer redemptions will each receive one (1) $5.00 TangoCard.com Reward. Rewards will be fulfilled via email within 30 days from the time the person competes a quote. The reward will be sent to the person in the form of a TangoCard.com Reward Link sent to the email address that received the quote offer reward email, which must also match the email address used when completing a quote for pet insurance. If a participant is not eligible to win or if Nationwide cannot contact the participant after a good faith effort, the reward will be forfeited.
4. Reward Recipient Criteria.
Reward recipients will be selected based on quotes completed during the Promotional Period which ends on the last day of the month that the quote offer email was sent at 11:59 PM PT. At the end of the Promotional Period, 1,000 reward recipients will be selected based on offer recipient eligibility and quote completion date and time.
5. Rewards
5.1 Rewards. The first 1,000 eligible offer recipients who complete a quote compliance to the criteria for reward offer at the end of the Promotional Period will each receive (1) $5 TangoCard.com Reward. ARV per recipient: $5. Total ARV: $5,000.
5.2 Expenses and Taxes. Reward recipients are responsible for any expenses incurred in connection with the rewards. No substitution or transfer of rewards is permitted. All federal, state, and local taxes on rewards are the sole responsibility of the reward recipient. Reward details not set forth herein are the sole discretion of Nationwide.
5.3 Legal Documents. If requested, reward recipients must execute and return an Affidavit of Eligibility, Liability Release, and (where legal) Publicity Release within three days of notification. Failure to do so may result in disqualification, forfeiture, or award to an alternate reward recipient.
6. Release.
All offer recipients who choose to participate in the quote offer promotion agree to release and hold harmless Nationwide, Tango Card, Inc and their respective subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies, and reward suppliers, and each of their respective parent companies and each such company's shareholders, officers, directors, employees, members, assigns and agents (collectively, the "Released Parties") from and against any claim or cause of action, including, but not limited to, personal injury, death, or damage to or loss of property, arising out of participation in the Promotion or receipt or use or misuse of any reward.
7. General Rules.
7.1. Applicable Laws. This promotion is governed by and subject to the laws of the United States and the state of Ohio without giving effect to the conflict of laws rules thereof. All federal, state, and local laws and regulations apply. The Promotion is void where prohibited by law. The reward winner may receive an IRS 1099 form for the value of their rewards. Participants agree to be bound by the Official Rules and the judges' decisions, which are final and binding on all matters relating to Promotions.
7.2. No Responsibility. Nationwide, and each of its respective affiliates, subsidiaries, parent corporations, and their respective officers, directors, shareholders, employees, and agents, are not responsible for any incorrect or inaccurate entry information, theft, tampering, destruction, or unauthorized access to, or alteration of entries; printing or other errors; and any entries or mail which are late, lost, delayed, incomplete, misdirected, stolen, mutilated, illegible, postage-due, or any combination of the above.
7.3. Nationwide's Rights. Nationwide can void any participant’s entry who it believes has attempted to tamper with or impair Promotion administration, security, fairness, or proper play. Nationwide reserves the right to modify, suspend or terminate the Promotion if necessary due to circumstances beyond Nationwide's control. In the event the Promotion is terminated early, rewards will still be granted through the date of termination.
7.4 Reward Link Terms and Conditions. Reward Link is managed by Tango Card, Inc. Reward Links expire on the date stated on the Reward and are valid within the USA only. Reward Links are issued to Reward Recipient without charge for loyalty, award, or promotional purposes, and are not sold to consumers. Your Reward Link balance may be redeemed for national gift cards and can be accessed at any time by clicking the Reward Link URL. Reward can only be redeemed by the reward recipient; Reward cannot be transferred, sold or exchanged. All redemptions are final. Reward Link balances from different programs cannot be combined. No fees apply to Reward Link. Please treat like cash. See full terms of service at https://www.rewardsgenius.com/reward-link-terms-of-service. Use of this Reward constitutes acceptance of the Reward Link terms of service. Separate terms and conditions apply after Reward Link redemption based on merchant selected for redemption.
8. Governing Law and Disputes.
8.1. Governing Law. This Promotion is governed by and subject to the laws of the United States and the state of Ohio without giving effect to the conflict of laws rules thereof.
8.2. Disputes. Except where prohibited, the participant agrees that any and all disputes that cannot be resolved between the parties, and all claims and causes of action arising out of or connected with the Promotion or any reward awarded, or the determination of winner shall be resolved individually, without resort to any form of class action. Further, in any such dispute, under no circumstances will participant be permitted to obtain awards for and hereby waives all rights to claim punitive, incidental, or consequential damages or any other damages, including attorneys' fees. Participants further waive all rights to have damages multiplied or increased.
9. Privacy.
By entering into this Promotion, you also agree to Nationwide's use of your personal information as described in the Nationwide Privacy Policy at http://www.petinsurance.com/privacy.aspx.
10. Winner List.

11. Abbreviated Rules.
No purchase is necessary. Promotional period ends on the last day of the month that the quote offer email was sent at 11:59 PM PST. Promotion open to legal residents of U.S. who are at least 18, except where prohibited by law. Invitation required for Promotion participation.
